Hello, I'm Melissa Brubaker, a psychiatric nurse practitioner offering medication management services for adults only (18-64 years old) via telehealth across Pennsylvania.

I specialize in treating anxiety, depression, trauma/PTSD, OCD, and other mental health conditions. My goal is to support adults in building stability and lasting growth within a collaborative space.

I chose the name Rooted Mind Psychiatric Services, PLLC to reflect the belief that mental well-being is supported by many interconnected elements. Just as strong roots keep us grounded, we need a foundation of emotional, physical, and relational support to stay resilient and navigate challenges. My approach combines clinical expertise with genuine care. I integrate medication, supplements, lifestyle support, and practical coping strategies to meet your individual needs.

Psychiatric Evaluation

Psychiatric Evaluation

The initial psychiatric evaluation can take up to 60 to 75 minutes. We'll explore your story, concerns, and goals while reviewing symptoms, past treatments, and preferences. This helps us understand your needs and begin building a plan that feels meaningful and sustainable.

Medication Management

Medication Management

Medication management visits are up to 30 minutes and focus on monitoring symptoms, evaluating treatment response, and addressing any medication concerns. These sessions provide space to track progress, adjust as needed, and ensure your treatment continues to support your mental health.

Brief Psychotherapy

Brief Psychotherapy

Brief psychotherapy is integrated into medication management visits, offering space to process challenges, build insight, and strengthen coping skills. Sessions may include tools for managing anxiety, mood, stress, or life transitions, tailored to your needs and goals. Please note, I do not offer therapy only.

Who do you work with?

Adults ages 18 to 65 across Pennsylvania via telehealth only. I treat depression, anxiety, trauma/PTSD, OCD, and other mental health conditions.

What services do you offer?

Psychiatric evaluations, medication management, and brief supportive psychotherapy. I do not offer therapy alone. If you're not taking medication and are seeking therapy only, I can refer you to trusted providers.

Who is not appropriate for this practice?

This practice is not appropriate for individuals needing crisis support, on-call access, or intensive services. I do not provide care for those with recent suicide attempts, active suicidal or homicidal ideation, psychosis, severe substance use, or repeated hospitalizations. In these cases, I can refer you to a higher-level care.

Do you accept insurance? Do you have cash pay options?

Yes. I accept several insurance plans through Headway (headway.co) to make care more accessible. These plans include - Aetna, Cigna, Capital Blue Cross, Independence Blue Cross, Oscar, Oxford, and United Healthcare (Optum). Cash pay is available: $200 for initial evaluations (60–75 mins) and $150 for follow-ups (25–30 mins).

What is your cancellation policy?

Cancellations require 24 hours' notice. Late cancellations incur a $50 fee.

How do sessions take place?

Sessions are held via secure video (doxy.me) and require both audio and visual. You must be located in Pennsylvania and not driving during the visit.

What should I expect for the first appointment?

After scheduling, you'll receive an intake packet with required forms and consents. These must be completed at least 24 hours before your appointment. The initial evaluation lasts 60–75 minutes and includes a thorough review of your symptoms, history, and goals. We'll discuss diagnosis, treatment options, and next steps.

Do you prescribe controlled substances?

No. I do not prescribe stimulants, benzodiazepines, or hypnotics due to current and pending DEA telehealth regulations.

How do I contact you outside of visits?

After intake, communication will occur through secure messaging platforms. Full details are provided during your evaluation and in the policy packet.

Are there forms or documentation not completed by this practice?

Yes. This practice does not complete forms related to disability, worker's compensation, forensic matters, or emotional support animals.

What if I have an emergency or urgent need?

I do not offer after-hours or crisis services. If you are experiencing a mental health crisis or emergency, call 911 immediately, or dial 988 for the Suicide & Crisis Lifeline, or go to the nearest emergency room.

Will I receive detailed policies?

Yes. A full policy packet is provided after an intake is scheduled.
