Hello, I’m Melissa Brubaker, a psychiatric nurse practitioner and founder of Rooted Mind Psychiatric Services.
I provide medication management for adults (18–64 years old) across Pennsylvania, helping people from all walks of life build stability and meaningful growth.
Just like trees rely on strong roots to stay grounded, mental health depends on a solid foundation, especially when facing depression, anxiety, or other mood concerns. I support that foundation through thoughtful medication management when appropriate, while treating you as a whole person, not just a diagnosis. I also provide brief, practical guidance and strategies to help you navigate daily challenges and maintain balance.

VIEW SERVICES
The initial evaluation is 60-90 minutes. I’ll explore your concerns, past experiences, and personal goals to understand your needs and begin building a plan that feels meaningful and sustainable to you. * You’ll receive an intake packet to be completed at least 24 hours before your appointment to help gather background information and guide our discussion.
Follow-up visits are up to 30 minutes and focus on tracking your progress, addressing any medication concerns, and adjusting treatment as needed to support your mental health goals. I also support deprescribing when appropriate, evaluating risks and making gradual, monitored changes.
During medication visits, I may provide guidance on daily habits like sleep, nutrition, supplements, and coping strategies, and offer brief support to help you process concerns, consider next steps, and explore solutions for managing daily stressors. * These sessions are not comprehensive therapy; ongoing therapy is best provided by a separate therapist.

Please note: Virtual visits legally require you to be in Pennsylvania at the time of the visit.
This practice is not a good fit for individuals needing immediate crisis support, on-call availability, or intensive services. This includes active suicidal or homicidal ideation with plan and intent, psychosis, severe substance use, or frequent psychiatric hospitalizations. These concerns are best supported in settings with more comprehensive resources, and I can help direct you to appropriate care.

Insurance is accepted through Headway (headway.co) with coverage available through Aetna, Capital Blue Cross Pennsylvania, Carelon Behavioral Health, Cigna, Highmark Blue Cross Blue Shield Pennsylvania, Independence Blue Cross (VIRTUAL ONLY), Oscar, Oxford, and United Healthcare (Optum). Self-pay is also available for those who prefer not to use insurance, with fees of $225 for the initial evaluation (60–90 minutes) and $150 for follow-up visits (30 minutes).
Sessions are currently held via secure video (Doxy.me) with both audio and visual required. You must be located in Pennsylvania during the visit. In-person appointments may be offered in the future.
Cancellations require 24 hours’ notice. Late cancellations or no-shows incur a $50 fee. You may reschedule, but rescheduling within 24 hours does not waive the fee.
Controlled substances such as stimulants/ADHD medications, benzodiazepines, or hypnotics, are not being prescribed by this practice at this time.
Disability, worker’s compensation, forensic, and emotional support animal documentation are not provided.
