About Me

Hello, I’m Melissa Brubaker, a psychiatric nurse practitioner and founder of Rooted Mind Psychiatric Services.

I provide telehealth psychiatric medication management for adults in Philadelphia and throughout Pennsylvania. I strive to offer affirming care for individuals of all backgrounds and identities.

Just as trees rely on strong roots to stay grounded, mental health depends on a solid foundation, especially during periods of overwhelm, sadness, or significant stressors. Whether you are experiencing anxiety, depression, PTSD/trauma, OCD, or other mental health concerns, my role is to help strengthen your roots through thoughtful medication management.

In addition to medication, brief therapy and practical interventions may be incorporated into care to help you build routines, set achievable goals, and develop strategies that support mood, energy, and overall well-being. Sessions are collaborative and focused on sustainable progress in everyday life.

At Rooted Mind, you do not have to navigate mental health challenges alone. I’ll be alongside you as we work toward meaningful and sustainable change.

Mental Health Evaluation

Psychiatric Evaluation

Your initial 60–75 minute evaluation is an opportunity for us to explore your concerns, contributing factors, and goals, and for me to better understand your needs in order to develop a meaningful, personalized treatment plan together.

* You’ll receive an intake packet to complete at least 24 hours before your appointment to help gather background information and guide our discussion.

Medication Management

Medication Management

Your 30-minute follow-up visit is an opportunity to monitor your progress, address medication concerns, and adjust treatment as needed to support your mental health goals.

When indicated, lab studies, GeneSight testing, and discussion of supplements or lifestyle factors may be incorporated into your treatment plan. I also assist with deprescribing when appropriate by carefully evaluating risks and making gradual, monitored changes.

Brief Therapy

Supportive Guidance

Brief therapy and supportive interventions may be incorporated into your 30-minute follow-up visit from a whole-person perspective. This may include discussion of habits such as sleep, nutrition, and routines, as well as helping you process concerns, clarify next steps, and explore practical ways to manage stress and improve functioning.

* Please note, this is not a replacement for ongoing comprehensive therapy with a separate therapist.

Do you have availability / are you accepting new patients?

Yes! I am currently accepting new patients. Due to platform limitations, online booking only shows availability within the next two weeks. If you don't see a time that works for you, please reach out by phone, email, or the contact form below — I'm happy to schedule further out.


Are my services right for you?

I work with adults ages 18 to 64 across Pennsylvania, providing medication management with practical supportive guidance for daily life. I treat anxiety, depression, PTSD/trauma, OCD, and other mood-related concerns.


How do sessions take place?

Sessions are currently held via secure video (Doxy.me) with both audio and visual required. You must be located in Pennsylvania during the visit. In-person appointments may be offered in the future.


Do you accept insurance? Do you have self-pay options?

Yes, I accept most major insurance plans through Headway*(headway.co) including Aetna, BCBS/Highmark, Cigna, Independence Blue Cross, Oscar, Oxford, and United Healthcare (Optum). Self-pay is also available: $225 for 60-75 minute initial evaluations, and $150 for 30 minutes follow-up visits. All payments are processed securely through Headway*. * What is Headway?

Headway is a secure, independent platform I partner with to make insurance easier to navigate. It handles insurance billing, benefit verification, and payments. To get started, you’ll create a profile and upload your insurance details, and Headway will confirm your coverage and share an estimate of your costs before scheduling. Protecting your privacy is very important to me, so all clinical notes are kept in a separate, secure electronic health record system and are only shared when necessary, such as for insurance audits or with your permission.


Do you prescribe controlled substances?

This practice does not prescribe controlled substances such as benzodiazepines or hypnotics. Stimulants may be prescribed when medically indicated.


Do you diagnose and treat ADHD?

ADHD support and treatment require a recent neuropsychological evaluation confirming the diagnosis. Prescriptions may not be provided on the same day if additional information, such as medical history or other documentation, is needed to ensure safe treatment. Non-stimulant options, such as Strattera or Wellbutrin, may also be considered. Stimulants will not be prescribed if you are taking other controlled substances or using illicit substances.


Do you offer therapy alone?

While brief therapy and supportive interventions may be incorporated into follow-up visits, my practice primarily focuses on medication management rather than standalone ongoing therapy. If you’re seeking therapy only, I’m happy to refer you to trusted providers.


Who is not appropriate for this practice?

This practice is not a good fit for individuals needing immediate crisis support, on-call availability, or intensive services. This includes active suicidal or homicidal thoughts with plan and intent, psychosis, severe substance use, or frequent psychiatric hospitalizations. These concerns are best supported in settings with more comprehensive resources, and I can help direct you to appropriate care.


What is your cancellation policy?

Cancellations require 24 hours’ notice. Late cancellations, rescheduling or no-shows incur a $50 fee.


Are there forms or documentation not completed by this practice?

Disability, worker’s compensation, forensic, and emotional support animal documentation are not provided.


What if I have an emergency or urgent need?

I do not offer after-hours or crisis services. If you are experiencing a mental health crisis or emergency, please seek immediate support using the resources below:
